Back Pain Referrals

11/14/2015

Comments

Refer your chronic or new back pain patients to GW's Spine and Pain Center with an easy to use referral sheet. The Spine & Pain Office will call patients at home to schedule an appointment and work with them on insurance pre-authorization patients. He says they can typically see patients within one week.

For you to do:
1. Print the form below and fill it out. To better use your time, have the patient fill out their name, address and insurance information.
2. Fax your ED Note (or ask the unit secretary), any relevant imaging reports and the referral sheet to the number at the top of the referral sheet. This is the most important part.
3. Remind your patients to expect a call from the Spine Clinic and that not all insurance is accepted (they do take two forms of DC Medicaid) but TriCare and a sub-type of Blue Cross has been an issue.

The referral sheets are available above the prescription printing are on the Urgent Care side. Or search your email for this note and print the attachment.
