All of your post-concussive patients can now follow up with the new MFA concussion clinic. They should be given the neurology follow up information and told to specify that they want the concussion clinic when calling for scheduling.
To qualify for APRA, your patient must have a DC license/identification. Treatment includes assessment and referrals for appropriate levels of substance abuse treatment including outpatient, intensive outpatient, residential, detoxification and stabilization, and medication assisted therapy. Recovery Support includes wrap-around services, such as care coordination, mentoring, coaching, educational support, job readiness and training, public transportation and other services to support recovery. Refer your chronic or new back pain patients to GW's Spine and Pain Center with an easy to use referral sheet. The Spine & Pain Office will call patients at home to schedule an appointment and work with them on insurance pre-authorization patients. He says they can typically see patients within one week. For you to do: 1. Print the form below and fill it out. To better use your time, have the patient fill out their name, address and insurance information. 2. Fax your ED Note (or ask the unit secretary), any relevant imaging reports and the referral sheet to the number at the top of the referral sheet. This is the most important part. 3. Remind your patients to expect a call from the Spine Clinic and that not all insurance is accepted (they do take two forms of DC Medicaid) but TriCare and a sub-type of Blue Cross has been an issue.
